Why does doubling distance quarter the dose rate?

For an idealized point source, radiation spreads across the surface of a sphere. Since the area is $4\\pi r^2$, the same emission is distributed across four times the area when distance doubles. Which assumptions have to be true before we use this relationship in a protection calculation?
